# Could an ICHRA Support Your Benefits Strategy?

Providing health benefits has always been a challenge for employers. Rising costs, changing regulations, and a wider range of benefit options have made the decision even more complex.

One option receiving increased attention is the **Individual Coverage Health Reimbursement Arrangement**, or ICHRA. This approach can offer more predictable costs and greater flexibility than a traditional group health plan. However, many employers are still evaluating whether it is the right fit for their workforce.

## What Is an ICHRA?

An ICHRA allows an employer to reimburse eligible employees for qualifying individual health insurance premiums. When the arrangement meets applicable IRS and Affordable Care Act (ACA) requirements, those reimbursements are generally tax-free. Depending on the plan design, certain medical expenses may also qualify for reimbursement.

Employees who participate must enroll in eligible individual health insurance coverage, which may include Medicare. Employers generally decide the reimbursement amount, subject to regulatory requirements.

## Potential Advantages

An ICHRA can provide several potential benefits for employers.

First, it may create more predictable healthcare costs. Instead of managing a traditional group plan, employers commit to a defined reimbursement amount for eligible employees.

Second, employees choose the individual health insurance coverage that best meets their needs. Coverage may be purchased through private insurers or through a Health Insurance Marketplace established under the ACA.

Third, employers may have flexibility in determining which employee groups are eligible for an ICHRA, although federal rules govern how employees can be classified. An employee generally cannot be offered both an ICHRA and a traditional group health plan at the same time.

## Growing Interest Among Employers

Interest in ICHRAs continues to grow.

According to a July 2026 survey published by the Employee Benefit Research Institute and Morgan Health, more than one-third of surveyed employers said they were evaluating or actively planning an ICHRA. However, only 11% reported actively planning implementation, suggesting that many organizations remain in the exploratory stage.

Among employers already offering health benefits, one-third indicated they would likely adopt an ICHRA within two years after learning more about the arrangement. Interest was strongest among organizations with at least 100 employees.

The survey also found that affordability, flexibility in plan design, and increased employee choice were among the most common reasons employers would consider an ICHRA.

## Important Considerations

While the potential advantages are appealing, an ICHRA is not automatically a simpler or less expensive alternative to group health insurance.

The effectiveness of an ICHRA often depends on the individual insurance market available to employees. Employers surveyed expressed concerns about premium costs, out-of-pocket expenses, provider networks, plan quality, and employees’ ability to select appropriate coverage.

Compliance and administration are also important considerations. Employers cited concerns about implementation complexity, ongoing administration, and the resources required to manage the program.

Tax consequences should be reviewed as well. In some situations, employees participating in an ICHRA may not qualify for premium tax credits available through the Health Insurance Marketplace. Applicable large employers should also evaluate how an ICHRA interacts with ACA employer shared responsibility requirements.

## Is an ICHRA Right for Your Organization?

An ICHRA may be a valuable component of an overall benefits strategy, particularly for organizations seeking greater cost predictability and flexibility. However, the decision requires careful analysis of employee needs, market conditions, compliance requirements, and financial implications.

A thoughtful evaluation can help determine whether an ICHRA aligns with your organization’s goals and workforce needs. At Contempo HCM, we believe informed decisions start with a clear understanding of both the opportunities and the tradeoffs.
